About Natasha Mack

LPN for close to 20 years. The majority of my experience has been in the primary care setting. Working in primary care has allowed for me to expand my skill set beyond the traditional LPN responsibilities. For the last five years I have been working as a chronic disease coordinator providing patient outreach with an emphasis on reducing repeat hospital admissions or high emergency department utilization. In this role I have had the chance to impact patients that are often in need of more support to address not only chronic disease but socioeconomic barriers impacting their care. As this role is remote I have been able to sharpen my prioritization, adaptability, and independent thinking skills. Although I am not face to face with patients or the providers I have been able to build trust and rapport. Since being in this role I have consistently met or exceeded expected outreach metrics and overall reduction of hospital readmissions.
