Our SW team empowers clients and their loved ones to identify long term care needs, pro-actively plan to secure personal and professional networks of help, learn strategies for decreasing stress and cultivating wellness. Responsibilities include staff supervision, provision of elder care consultations, individual and family counseling, information and referral services, caregiver support group facilitation, crisis intervention, conflict mediation, patient care advocacy, community engagement and partnerships to create intergenerational dialogue with Stanford U staff, scholars, Fellows, in Medicine, public speaker, hosts educational forums. During my tenure have expanded Avenidas social work services; in 2016-2019 awarded new caregiver education and emergency fund grants, coordinated substantive press coverage with our marketing team: raised the profile of Avenidas Care Partners within the Santa Clara County elder care provider network. Community partners include Kaiser Health Care, the VA Hospital, the Palo Alto Medical Foundation, Stanford University Hospital and Medical School, the Alzheimer's Association, UCSF Aging and Memory Clinic, Santa Clara County Aging Collaborative. Received Stanford Community Engagement Award 2022 for agency partnership with Mona Hartmann, Child and Elder Care Services Program Manager, for outreach and services provided to Stanford family caregiver staff.
