We provide geriatric house call primary care services, chronic disease management, medication management/review, palliative care, assistance with advance directives/goals of care and more-right in our clients' own homes. ALF, SNF, LTC, independent living. We have mobile urgent care, mobile imaging, mobile lab services, mental health providers. Our commitment is to provide patients with more quality time with their provider. Our geriatric patients and their families deserve strong advocacy and guidance to navigate a very complicated healthcare system. We are committed to spending more time with patients and families, so they feel they are heard and receiving consistent, quality care in their own homes.
