Our mission is your mission. We enable health and healthcare delivery professionals to access and share critical patient health information to provide better care. We are committed to improving the health of our communities through the use of data, collaboration and health improvement strategies that work! We develop technologies to increase efficiency and reduce cost. We actively support public health initiatives. We create partnerships. We aggregate and organize data. We facilitate collaboration, train, and advise. Our health information exchange network serves over 9.5m patients cared for by 1600+ participant organization members in the central new york, north country, southern tier and hudson valley regions of NY. Through trusted collaboration and valued services, HealtheConnections enables improvement in health and healthcare delivery that results in healthier people, better care, and efficient cost. The name HealtheConnections represents our commitment to connect, engage and collaborate with health care physicians, hospitals, public health, mental and behavior health and human services, and other health and health care providing entities, along with insurers, businesses and consumers, to deliver regionally valued services and initiatives which support New York state’s health information exchange and population health improvement agendas.
