As a Nursing and Program Manager, I have over 19 years of experience in providing high-quality care to patients across various settings and specialties, with a focus on cardiac nursing and patient advocacy. I hold a Bachelor of Science in Nursing from Grand Valley State University and I am passionate about improving health outcomes and patient satisfaction. In my most past roles, I lead the design and implementation of innovative post-acute care programs that reduce readmission rates and close gaps in care transitions. Some of the programs I have successfully launched include remote patient monitoring, transitional heart failure care, and care transition nurse programs. I collaborate with a multidisciplinary team of clinicians, administrators, and community partners to ensure the delivery of evidence-based, patient-centered, and cost-effective care.
