Hi! I've been a nurse for 36 years with the majority of my career spent in home care and hospice. For the past few years I have migrated to case management. I had no idea how many different settings and roles case managers play. I obtained my CCM last year. I started out as a disease case manager and am currently working in a collaboration between Cleveland Clinic and Aetna insurance.
