My objective is to ensure quality living, working, and educational opportunities for children and adults with disabilities within their community. PROFILE * Home and Community Based Training Specialist, providing area-wide training for HCBS community service providers on: Rights and Responsibilities, Abuse, Neglect, and Exploitation, Behavioral Support Planning, Person-Centered Support Planning, Assessment of Risk, and Prader-Willi Syndrome Supports. • Over 22 years of social service experience as a Targeted Case Manager, Recreational Lead, and Direct Support Professional for adults and children with disabilities. • Working knowledge of service coordination, including: Person-centered planning, positive behavioral support development, implementation of supports, needs assessment development, and recreational therapy. Experience in plan development and supports for persons with genetic conditions such as Klinefelter Syndrome and "XXYY" Syndrome. • Experience with planning, and implementation of supports for those diagnosed with Fetal Alcohol Syndrome. • Specializing in serving consumers with dual diagnosis (I/DD & MI) facing behavioral and substance abuse challenges: Addressing risks to health, welfare, and safety. • Created and managed numerous cultural awareness and promotional programs including donation drives, art exhibits, volunteer recruitment, and special classes for developmentally-disabled adults. • Specialized training in Prader-Willi Syndrome supports through the Prader-Willi Syndrome Program at The Children’s Institute in Pittsburg, PA.
