I work delivering support coordination and mental health services based in Newcastle and Lake Macquarie (travel role). Supporting clients to determine goals and objectives, develop mental and physical support plans while promoting choice and control.Coordinating mainstream, informal and formal supports with clients to assist them in achieving their goals.Establish a respectful and collaborative working relationship with clients and their support network that encourages capacity building and promotes choice, control and resilience.Develop strong and collaborative working relationships with relevant internal and external stakeholders including NDIA Communities and Justice, health, education, employment along with Community NFPs and other mental health service providers to ensure effective support coordination.Support and assist clients to de-escalate and access services during crisis or periods of escalation of needs.Regularly monitor funding to ensure services are being delivered within the services allocated budget. Monitor and report progress against goal plans and budgets.Engage with regional contacts, Local Area Coordinators, menta health and crisis prevention organisations to manage triaged and assessed risk amongst client(s).Compliance with code of conduct, policies and procedures and relevant government legislation, standards, rules and guidelines where relevant.Maintain the privacy and confidentiality of clients, families and the organisation
