About Teresa Gray

With over 19 years of experience as a physician, I am passionate about providing high-quality, patient-centered, and evidence-based care to adults with complex medical needs. I am board certified in Internal Medicine and Obesity Medicine, and have a strong interest in preventive medicine and ambulatory care. I currently work as a physician at HealthPartners, where I deliver primary care and management of chronic conditions in an outpatient clinic setting.In my role at HealthPartners, I collaborate with a multidisciplinary team of nurses, social workers, pharmacists, and specialists to provide personalized and coordinated care. I use telehealth and mobile technology to optimize access, communication, and documentation. I also participate in quality improvement initiatives and clinical education, to enhance the outcomes and satisfaction of our patients and staff. I have developed expertise in chronic care management, especially in the areas of diabetes, hypertension, obesity, and dementia. I utilize my skills in preventive medicine, ambulatory care, and obesity medicine to help my patients achieve their health and wellness goals.
