Motivated to assist participants achieve self-sufficiency, independence, build capacity and strive to reach their goals. I have a strong knowledge of SDA and SIL and as of present I have achieved 100% success rate with all applications submitted. I pride myself on achievements accomplished for the participants I support and have the confidence and drive to explore alternative supports. Previously I worked in the Aged Care Sector offering 5 years’ experience as a Program Coordinator/ HCP Case Manager building a team of professional support workers to provide support and assist clients to remain living at home for as long as possible and to enable them to have choice and flexibility in the way that they are managed. I am extremely person-centered and focused driven. I enjoy the benefit of working in a diverse team and meeting a broad range of people in the community. I have the ability to network, build partnerships and work collaboratively.
