Contact Wellness Foundation

Contact Wellness Foundation (CWF), an Oregon nonprofit Corporation, was formed in April, 2010, to improve Quality of Life by enabling additional opportunities for personal Health and Wellness. Our goal is to continually seek leading edge ideas with practical application to empower people living with diabetes to accept personal responsibility for their continued health and wellness – changes in attitudes, beliefs and behavior leading to improved self-management of one’s condition. Such applications will be supported with opportunities to build positive relationships through continued connection and online social networking with one’s peers. It is our desire to discover and define a Wellness Algorithm, that when coupled with effective instructional framework, will encourage personal responsibility that results in prevention, including outcomes of tight management and control of metabolic metrics, ultimately driving improved quality of life and reduced healthcare costs.
