I have extensive experience in performing comprehensive assessments within a multidisciplinary team assisting clients to build knowledge and skills to adapt to life post a life changing physical health event. I am able to undertake consistent stakeholder engagement and promotion of cardiac and rehabilitation services to GP's and referring acute hospitals. Enable safe and effective transitions across the care continuum and facilitate referrals to appropriate health and well-being resources and services. I have a solid work ethic, proven skills in co-ordinating and provision of rehabilitation and cardiac and respiratory services in individual and group settings.I have experience in delivering care planning and education that is underpinned by sound clinical knowledge.
